Sample Language for Your Will or Trust
To include a gift in your will that benefits your local Providence hospital, be sure to include this language:
“Providence (insert local hospital name here along with their city, state) with Federal Tax ID (insert the appropriate Tax ID number).”
*If you are interested in supporting a specific fund or service line, it is best to discuss your plans with our planned giving professionals before completing your bequest language to confirm your wishes can be met by the hospital.
